I have been advised to consult with a physician before beginning any exercise program, and I agree that all activities I undertake at the Studio are at my own risk. I will inform an instructor of any and all new or pre-existing conditions, limitations, or concerns prior to the start of any class or session. 3. Release and Waiver of Liability: I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless the Studio from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of the Studio's equipment or facilities, including any such claims which allege negligent acts or omissions of the Studio. 4. Indemnification and Hold Harmless: I agree to indemnify and hold the Studio harmless from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, brought as a result of my involvement at the Studio and to reimburse them for any such expenses incurred. 5. Photographic Release: I grant the Studio the right to take photographs or videos of me and my property in connection with the above-identified subject. I authorize the Studio, its assigns, and transferees to copyright, use, and publish the same in print and/or electronically.
